SELF CARE CENTER – OFFICIAL POLICY MANUAL

A Membership-Based Social Safety Net Program

SECTION 1 — INTRODUCTION & PURPOSE

Self Care Center (“SCC”) is a membership-based organization designed to empower and support individuals who experience unexpected illness, temporary disability, or inability to work, resulting in financial hardship.

SCC provides a financial social safety net benefit of $3,000 or $5,000 per month, for up to 2 months, depending on the premium level selected by the member.

SCC is not insurance. It is a community-supported protection program funded by member premiums and governed by these policies.

SECTION 2 — MEMBERSHIP POLICY

2.1 Eligibility

To qualify for membership, individuals must:

  • Be at least 18 years of age.
  • Live in the United States or Canada.
  • Complete the membership application and agree to all Self Care Center policies.
  • Pay the required monthly premium on time.

2.2 Membership Premium Options

Members may choose one of the following premium-based support programs:

  • $50 / month → Benefit: $3,000 per month, Maximum Duration: Up to 2 months
  • $99 / month → Benefit: $5,000 per month, Maximum Duration: Up to 2 months

2.3 Membership Waiting Period

A mandatory 90-day waiting period begins on the date the member applies and submits their first premium payment.

SECTION 3 — PREMIUM PAYMENT POLICY

3.1 Payment Calendar

All members must pay their monthly premium between the 1st and 10th of each month.

3.2 Late or Missed Payments

  • Failure to pay within 30 days results in automatic cancellation of membership.
  • No refunds will be issued for missed payments, partial payments, or cancellations due to non-payment.

SECTION 4 — SAFETY NET SUPPORT BENEFIT POLICY

4.1 Eligibility to Receive Benefits

Members may apply for benefits when all the following conditions are met:

  1. Membership has been active for at least 90 days.
  2. Member provides a Doctor’s Note confirming unexpected illness or medical condition that prevents them from working.
  3. Member is unable to work and unable to pay essential bills as a direct result of illness.

4.2 Benefit Amount

Depending on the premium level, members may receive:

  • $3,000 per month, up to 2 months, for $50/month.
  • $5,000 per month, up to 2 months, for $99/month.

4.3 Benefit Payment Timeline

Self Care Center will issue payments within 3 to 7 business days after receiving a complete and verified claim.

4.4 Required Documentation

Members must submit:

  • Completed Self Care Center Claim Form
  • Doctor’s certification
  • State ID and Member Number

4.5 Income Replacement Collection Policy

When benefits are approved:

  • Self Care Center will collect the member’s monthly premium directly from the benefit payments, since the benefit is replacing their income.

4.6 Restrictions

Benefits cannot be claimed for:

  • Cosmetic procedures
  • Non-medical reasons
  • Intentional injuries
  • Fraudulent claims (subject to contract termination)

SECTION 5 — CODE OF CONDUCT

Members must:

  • Provide truthful and accurate information at all times.
  • Treat clients with dignity and respect.
  • Not misuse or attempt to defraud the program.
  • Not submit altered medical documents.
  • Follow all Self Care Center policies and procedures.

Violation may result in:

  • Termination of membership
  • Denial of benefits
  • Legal action if fraud is involved

SECTION 6 — PRIVACY & DATA SECURITY POLICY

Self Care Center protects all members’ information, including:

  • Medical notes
  • Social Safety Net records
  • Contact information

Data will only be shared for:

  • Claim verification
  • Internal program administration

Important: Self Care Center does not sell or distribute members' information to third parties.

SECTION 7 — REFUND POLICY

No refunds will be issued for:

  • Paid premiums
  • Membership cancellations
  • Lapsed memberships due to non-payment
All premium payments are final, as they support the community safety pool.

SECTION 8 — CONTRACT RENEWAL & TERMINATION

8.1 Renewal

Memberships automatically renew annually unless the member opts out.

8.2 Member-Initiated Cancellation

Members cannot cancel at any time. Cancellation is allowed only after one year of renewal, and:

  • No refunds will be issued.
  • The waiting period resets if they return.

8.3 Self Care Center–Initiated Termination

Membership may be terminated by Self Care Center for the following reasons:

  • Fraud or attempted fraud
  • Abuse of benefits
  • Non-payment of premium for 30 days

SECTION 9 — CLAIMS APPEALS PROCESS

If a claim is denied, the member may:

  • Request an appeal within 15 days of the denial.
  • Provide additional documentation or clarification to support the appeal.

Appeals are reviewed within 10 business days.

SECTION 10 — AMENDMENTS

At the end of each year, Self Care Center will:

  • Analyze and evaluate areas for improvement.
  • Reserve the right to update policies, pricing, or benefits.

Members will be notified in writing at least 60 days before any policy changes take effect.
